LML Duramax — CP4.2 Pump Failure (Same Story, Different Engine)

The LML Duramax shares the same Bosch CP4.2 pump that's notorious on the 6.7L Power Stroke — and the same failure mode. When the LML CP4 fails, it sends metal through the entire fuel system. Here's the diagnostic, the fix, and the CP3 conversion option that prevents it.

What this issue is

The LML Duramax (2011-2016) shares the same Bosch CP4.2 high-pressure injection pump that’s notorious on the 6.7L Power Stroke — and the same well-documented failure mode. The cam-and-roller wear pattern that liberates metal into the fuel system happens on both platforms for the same underlying reason: US ultra-low-sulfur diesel has reduced lubricity compared to what the CP4 was originally designed around.

When the LML CP4 fails, it doesn’t fail quietly. Metal shavings from the failing pump’s internal contact surfaces circulate through the entire fuel system — fuel lines, both fuel rails, all eight injectors, pressure sensors, fuel filter housing. The repair scope is full fuel system replacement.

Symptoms you’ll notice

  • Hard start or no-start — the truck struggles to build the rail pressure needed to fire
  • Low fuel pressure codes — P0087 (fuel rail pressure too low), P0088 (fuel rail pressure too high), and various P008X variants
  • Metallic debris visible at the fuel filter or in a fuel sample drawn from the system
  • Surging or loss of power under acceleration when the rail pressure can’t keep up with injector demand
  • Check Engine Light with fuel-system-related codes
  • Stalling at speed as fuel system contamination progresses
  • Reduced fuel economy in the weeks or months preceding outright failure

The “before” pattern matters. Most LML owners get warning signs — slight performance degradation, occasional hard-starts, fuel economy creeping down — that get ignored until the pump fully fails. Catching the symptoms early lets us pull a fuel sample and check for metal contamination before the cascade.

Why it happens

Same root cause as the 6.7L Power Stroke CP4 failures. The Bosch CP4.2 pump’s internal cam and roller assembly depends on diesel fuel as its lubricant — there’s no separate oil supply. The cam runs against the roller continuously while the engine is running, generating the high pressures the common-rail system needs.

US ultra-low-sulfur diesel has had lubricity additives stripped out as part of the desulfurization process. The fuel lubricates the cam-roller interface less effectively than what the CP4 was originally designed and tested around. Over many thousands of operating hours, the cam and roller surfaces wear at a rate above design spec. Eventually metal-on-metal contact begins, and metal debris is liberated into the fuel system.

Once that debris is in the fuel, it circulates throughout the high-pressure side and damages everything downstream — injectors, rails, sensors. The pump that started the cascade may still be running when the truck arrives at the shop, but the damage is system-wide.

How we diagnose it

Sequence we use:

  1. Pull stored codes. P0087 and P0088 (with various extensions) are the primary indicators. Other fuel-system codes (rail pressure-related, injector codes) may be paired.

  2. Fuel sample inspection. Pull a fuel sample from the fuel filter or rail and inspect visually for metallic debris. Even small amounts of metal — glints visible against a white background, or magnetic particles attracted to a magnet — confirm CP4 failure is in progress.

  3. Fuel rail pressure live data. With the factory GM scan tool, monitor commanded versus actual rail pressure under various conditions. Significant lag of actual behind commanded, or actual that can’t reach commanded under load, points at high-pressure system failure.

  4. Injector return flow test. Measure fuel return from each injector. Wildly inconsistent return rates between injectors suggest either injector contamination from CP4 debris or independent injector failure.

  5. VCV (volume control valve) inspection. When we have access to the CP4, we inspect the VCV bore for metallic debris accumulation. This is the diagnostic step that confirms the pump is actively shedding material into the fuel.

If any of steps 2-5 return positive findings, we recommend full fuel system replacement rather than partial repair. Past experience: partial repairs on contaminated systems fail again within months as residual debris finds its way through the new parts.

What the repair looks like

Full fuel system replacement is the standard fix. Components:

  • CP4 pump replacement OR CP3 conversion to a CP3-based system (recommended for most LML owners — eliminates the failure mode)
  • All 8 injectors replaced
  • Both high-pressure fuel rails replaced
  • Fuel lines from tank to rail replaced
  • Fuel filter housing and filter replaced
  • Fuel tank cleaned thoroughly or replaced if contamination is severe
  • Sending unit replaced
  • Full system bleed and pressure test post-install

Should I do a CP3 conversion instead of replacing the CP4? +

For most LML owners, yes — and it's the preventative move we recommend before the CP4 fails in the first place. The CP3 conversion (S&S Diesel and Industrial Injection both make LML CP4-to-CP3 kits) swaps the failure-prone CP4 for the older Bosch CP3 pump, which is mechanically more robust and tolerant of US ultra-low-sulfur diesel. It eliminates the failure mode rather than just replacing the failed component.
