P0087 on the 6.4L Power Stroke means the fuel system can't build or maintain rail pressure under load. It can be something simple (restricted filter, sensor) or something major (injectors, high-pressure pump). Here's how we narrow it down without throwing parts at it.
What this issue is
P0087 on the 6.4L Power Stroke — “Fuel Rail / System Pressure Too Low” — means the engine can’t hold rail pressure at what the ECM is commanding. The 6.4L runs a common-rail diesel fuel system — a high-pressure pump feeds the fuel rail, rail pressure sensors monitor what’s happening, and each cylinder has a piezo injector that doses fuel on demand. When anything in that chain can’t keep up, P0087 is one of the flag codes.
The hard part of P0087 is that it doesn’t specify which component is the cause. It says “pressure is falling short,” not “injector three is leaking” or “high-pressure pump is tired.” That means accurate diagnostic work is the difference between fixing the truck right the first time and spending more than you needed to on parts that weren’t the problem.
Symptoms you’ll notice
- Check Engine Light with P0087 — the trigger code, often paired with related fuel-pressure codes (P008A, P0088)
- Loss of power under load, especially when towing or accelerating hard — the engine can’t get enough fuel to make rated power
- Extended cranking before firing — cold start and hot start can both be affected, depending on the root cause
- Stumble, hesitation, or surge under load — fuel pressure swinging around target because the system can’t hold it steady
- Limp mode triggered in severe cases — the ECM backs down power to protect the engine when pressure is well below command
- Rough idle in some cases — depends on which component is causing the pressure loss
- Hard hot-start or no-start in advanced cases — severe loss of fuel system integrity
- Paired codes — often P008A (low-pressure fuel system pressure too low), P0088 (fuel rail pressure too high), or injector-specific codes
If you’re towing and the truck goes into limp mode with P0087, get off the road and get it towed. Continuing to drive on P0087 under load when pressure is actively falling short is how modest repairs become major ones.
Why it happens
1. Restricted fuel filter. The simplest cause — a plugged filter restricts supply-side fuel to the high-pressure pump. If your fuel filter is overdue for service, this is the first suspect.
2. Failing lift pump or supply-side restriction. The 6.4L uses a lift pump to deliver fuel from the tank to the high-pressure pump. A weak lift pump, clogged pickup, or restricted supply line starves the high-pressure side and triggers P0087.
3. Failing rail pressure sensor. The sensor feeding pressure readings back to the ECM can fail or drift out of spec — the system may actually be delivering correct pressure but the ECM sees it as low.
4. Leaking injectors. Piezo injectors on the 6.4L can develop internal leaks that bleed pressure off the rail faster than the pump can replace it. Injector return-flow testing identifies which (if any) injectors are leaking.
5. Failing high-pressure pump. The pump itself can wear, losing its ability to build pressure. Not the most common cause early on, but a real one at high mileage.
6. Damaged or leaking rail or high-pressure lines. Less common — a cracked rail or leaking high-pressure line bleeds pressure externally. Usually visible during inspection.
7. Fuel system contamination. Water or debris in the fuel system damages components upstream to downstream. If there’s been a bad fuel event in the truck’s history, that affects the diagnostic path.
How we diagnose it
Sequence:

Pull all stored codes. P0087 alone tells us less than P0087 plus a specific injector code or a supply-side code. Related codes narrow the investigation.

Fuel filter and lift pump check. Simplest-to-eliminate gets checked first. Filter service history, filter condition, and lift pump pressure test.

Injector return flow test. With the return lines disconnected from each injector and fed into measuring containers, we run the engine and check how much fuel each injector is returning. Out-of-spec injectors are bleeding pressure.

Rail pressure monitoring. Live data from the rail pressure sensor under different conditions — cranking, idle, loaded run — tells us what the system is seeing versus what it’s commanding.

Supply pressure and volume test. Verify the high-pressure pump inlet is getting adequate supply pressure and volume.

High-pressure pump test. If upstream checks clear, we test the pump itself — pressure output, volume, leak rate.

Visual inspection for external leaks. Check all high-pressure fittings, lines, rail body, and injector cup areas for fuel leaks or residue.

What the repair looks like
Fuel filter service. Simple service — new filter, run through for codes, done. If this fixes P0087, you got lucky.
Lift pump replacement. Moderate scope — replace the pump, verify supply pressure is back to spec.
Rail pressure sensor replacement. Relatively modest — sensor is accessible, threads into the rail, replaced in under an hour with a proper scan tool reset.
Injector replacement (single or set). Significant scope — 6.4L injectors require substantial top-end access. Single-injector replacement is sometimes appropriate on lower-mileage trucks; full-set replacement is often the right call on higher-mileage trucks because the other seven aren’t far behind.
High-pressure pump replacement. Major scope — pump is in a demanding location and replacement is labor-intensive.
Rail or high-pressure line replacement. Specific to what’s damaged.
Full fuel system service after contamination event. When diagnostic identifies water or debris contamination, the correct repair is flushing the full system — replacing the pump, injectors, rail, filters, and lines — because contamination downstream of the pump contaminates everything. This is the most involved scenario and it’s driven by what’s in the fuel system, not by any single component failure.
